Welcome to the USCA LIFE Application

USCA LIFE is a non-degree seeking program for students with intellectual and/or developmental disabilities. This program does not result in a college diploma. Graduates of this program will receive an employment credential based on person-centered planning.

The USCA LIFE program is a two-year hybrid program that will:

Serve students with intellectual disabilities
Provide individual supports and services
Focus on academic and social inclusion
Provide access and skills for gainful employment
Integrate person-centered planning
Offer a meaningful credential.

In order to submit the USCA LIFE application, you will need to complete the following:

Complete the Personal Support Inventory (completed by parent/guardian)
Complete the Student Questionnaire (completed by student)
Submit contact information for 3 people using the Reference Form
Complete the Release & Exchange of Information Form
Pay the $45 application fee

After submitting your application, please upload the following items into your Pacer Portal:

Upload the student’s academic transcripts from high school (if classes were taken)
Upload the student’s most recent psychological evaluation (must be evaluated within last 3 years of application date)
Upload the student’s most recent Individualized Education Plan (last year in high school)

Please set aside at least one hour to complete the application.
